Price lists and order forms linked to this page are for Non-Michigan customers only.  Because of the major investment by Michigan departments and agencies in the development of the Michigan Model for Health^®, the Educational Materials Center made the commitment to discount prices to Michigan customers, hence the need for separate pricing structures.

Our Michigan Model for Health^® price lists are offered in Excel spreadsheets that will automatically calculate totals for you.  They can be a great tool when you're working within a budget to project costs as you compile your order.  Spreadsheets are three pages in length:
     Page 1 = Order Form and Pricing
     Page 2 = Order Summary and Type Codes legend
     Page 3 = Payment Information
