Price lists and order forms included on this page are for Michigan customers only.  Because of the investment by Michigan departments and agencies in the development of the Michigan Model for Health^®, the Educational Materials Center made the commitment to discount prices to Michigan customers, hence the need for separate pricing structures.

Our Michigan Model for Health^® price lists are offered in Excel spreadsheets that will automatically calculate totals for you.  They can be a great tool when you're working within a budget to project costs as you compile your order.  Spreadsheets are three pages in length:

     Page 3 = Payment Information
